Religious School Application
For Gan (K) through Kitah Hay (5th Grade) please fill out the following forms:
- 2011-12 Application for Grades Gan to Hay
- Class Placement Information Form (Optional)
- Special Needs Information Form (If Applicable)
New Online Registration Option for the 2011-12 School Year! For your convenience, instead of filling out the forms above, please consider using the online form for all Gan through Hay students.
Note: If you use the online option, you must still print out, complete and submit to the Religious School the Parental Authorization and Emergency Information Form.

Financial Assistance Information
If financial assistance is needed, please complete the Abatement Certification section of the Temple Emanuel Membership Booklet for the current year and submit it to Amy B. Klein, President or Michael Cantor, Executive Director, 617-558-8110.
